Individual Counseling

Individual Counseling is a process through which clients work one-on-one with a trained therapist—in a safe, caring, and confidential environment—to explore their feelings, beliefs, or behaviors, work through challenging or influential memories, identify aspects of their lives that they would like to change, better understand themselves and others, set personal goals, and work toward desired change.

People seek therapy for a wide variety of reasons, from coping with major life challenges or childhood trauma, to dealing with depression or anxiety, to simply desiring personal growth and greater self-knowledge. A client and therapist may work together for as few as five or six sessions or as long as several years, depending on the client’s unique needs and personal goals for therapy.

Nursing Services

Nursing services are rendered under a physician’s order for the purpose of monitoring, evaluating and assessing the physical problems and general wellness of particular individuals. In this process, our nurses conduct examinations, assessments and interventions that address physical, nutritional and behavioral health issues manifested in the course of the consumer’s treatment.

Medication Management

Medication Management offers initial assessment and on-going monitoring of medication needs associated with mental health and/or substance abuse challenges. Through coordination with other CitySide Healthcare treatment programs the prescribing practitioner is able to have increased insight into the individual’s treatment goals and progress allowing more thorough treatment.

Case Management

Case Management services consist of providing environmental support and care coordination considered essential to assist the individual with improving his/her functioning, gaining access to necessary services, and creating an environment that promotes recovery as identified in his/her Service Development Plan.

In addition, it can be effective in treating mental health concerns that impact the family as a whole, such as depression, substance abuse, chronic illness, and food issues, or everyday concerns, like communication problems, interpersonal conflict, or behavioral problems in children and adolescents.
Free Community Resource Referrals

Free community resource referrals are available and based on the identified needs of the client. A list of resources is compiled and provided to the client. These needs generally focus on nutrition, medical, mobility, housing and recovery needs. The client receives assistance in establishing contact with the resources and CitySide Healthcare staff follow through with the client to ensure connection has been made and the client is able to benefit from the resource referral.
